Name: (Name of your zanpaktou obviously, japanese name for shinigami/vaizards, spainsh name for arrancars)

Zanpaktou Spirit: (Describe or place a picture of what spirit your zanpaktou is based on, for resurreccion, the spirit must be an animal)

Element: (What element does you use? Fire, ice, water, etc)

Level: (For shinigamis, does your zanpaktou have just shikai or have you mastered bankai?)

Release Phrase: (What do you say to release your zanpaktou?)

Zanpaktou Description: (Describe or place a picture of what your zanpaktou looks like in the following forms: unreleased, shikai, and bankai, even if you havent attained bankai yet, still put a description so you dont have to worry it later)

Resurreccion Description: (Describe or place a picture of what you look like after you release your zanpaktou)

Ability: (What does your shikai and/or bankai, or resurreccion do when you release it, for example; does it give control of your element, does it give you a stat boost, etc)

Bankai Time Limit- (For shinigamis, admins will decide a time limit for how long you can stay in bankai state depending on your character's stats)
